The requirement applies to all passengers, regardless of nationality, including those transiting through the United States. The measure is designed to reduce the risk of Ebola entering the US, as the virus can take up to 21 days to cause symptoms, the airline said.
Each passenger must complete and sign a paper or digital declaration before boarding. Passengers who fail to submit the form will not be allowed to board, according to the CDC.
Transit through a DRC airport also counts as being in the country, even if the passenger does not leave the aircraft or airport. Children under 18 must have the declaration completed and signed by a parent, guardian or other authorized person.
Knowingly providing false or misleading information may result in criminal fines or imprisonment under US law.
Uzbekistan Airways urged passengers planning trips to the United States to review the new requirements and the declaration form in advance.
